Sister Irene (Farmiga) is now living a quiet life in Italy after the horrors of Saint Carthas monastery four years prior, but is still experiencing troubling visions. When a disturbing death is discovered in Tarascon, France, Irene is requested at the behest of the Cardinal (David Horovitch) to investigate the matter. Her findings lead her right back to the evil demon Valak (Bonnie Aarons), and Irene has no choice but to face Valak once more before anyone else succumbs to its possession.
